entitled, and for statistical purposes. This information will be disclosed to other governmental agencies subject to the restrictions of federal and
state requirements governing confidential information. If you do not provide your Social Security number, your claim for Unemployment Compensation
benefits will not processed.
Confidential UC information about you may be requested and utilized for other governmental purposes, including, but not limited to, verification of eligibility under other government programs.
Access to confidential information is restricted to authorized entities associated with providing services and performance under the Workforce Innovation and Opportunity Act of 2014.
